Equipment Modification RMA Request

To better serve our customers ASU has implemented a Return Authorization (RA) process to ensure the most efficient NVIS equipment modifications and repair.

Before sending equipment to ASU, customers are required to complete the following form for the service(s) requested. After submitting this form, ASU will provide you with an RA number and estimate for the repair or modification within one business day via email.

Important info about your shipment:

  • The RA number MUST be visible on the outside of the shipping box
  • Include a copy of the RA worksheet in the box
