To ensure priority service of your equipment, please use this return authorization form for NVG repair, 180-day certification, trade-up evaluation, or Hoffman 126-A Test Set calibration. This form will notify the NVG Service Center of your intent to send equipment for service. You will receive a Return Authorization number by email within one business day.

Before shipping your equipment: display the RA number on the outside of the box and include a completed Inventory of Items worksheet in your shipment.
